W-055: ecs-serialize entity references and codecs (SER-F03)

Goal

Implement refs fields and { trait, codec } entries on the save side: ctx.ref, dangling-ref for references to entities that are not saved (nothing is saved partially, OQ5), and codec-error wrapping of codec exceptions. The restore side (ctx.entity, codec fromJSON, round trips SER-036 and SER-037, and the restore half of SER-038) needs restoreWorld, so it is done in W-056.
